Insurance and Payments

For our patients’ convenience, DerVan Healthcare Services accepts insurance plans and other modes of payment, including:

Medicaid

United Healthcare

Please provide your accurate billing information for us to file your insurance claims. However, we may ask you to pay a co-insurance, deductible, or co-payment to proceed with our services. Our clinic accepts, cash, MasterCard, checks, and Visa.

Collection Policy

For patients with deductible insurance plans, we will collect an upfront fee of $100. Please call your insurance provider to determine if your plan has a deductible and how much the deductible is. This policy does not cover annual physicals. Please review your last physical under and how often does your insurance plan allows it.

What Is a Deductible?

In insurance contracts, a deductible is the amount paid for medical expenses as agreed by the insurance holder and insurance provider. Before the insurance plan covers healthcare costs, one must pay off the deductible.

Our clinic collects a deductible of $100 as an upfront fee. However, we may lift the collection if you can show us that you have met your annual deductible.
